Expect the scheduling process to take at least 10 minutes. Not only do we need to ask you several questions, but we want to have adequate time to address your concerns.
Have your referring physician complete a referral form .
Complete a registration form or have all insurance and/or claim information available.
List of participating Insurances

Please bring the following to your scheduled appointment

MRI Films, X-Rays Reports
List of all Medications that you are currently taking
Your insurance cards and Driver's license card or photo ID
Referral from your physician
Names and addresses of referring and primary doctors
