AND BILLING POLICIES:(PDF)
PLEASE NOTE: For any procedure done at IPM office your insurance
will be charged and you may have coinsurance from that bill.
Health Insurance:
Dr.Patel participates in many but not all health plans. We will
bill participating health plans, but please be aware that insurance
coverage is not a guarantee of payment. We will contact your insurance
to confirm coverage and benefits, but IPM cannot guarantee that
your insurance will pay for our services.
If your insurance does not pay, you will be responsible to pay your
bill in a timely manner.
Patient Financial Responsibility After Insurance Payment:
Most insurance companies require the patient to pay for a portion
of their medical care. The nature and amount of your outofpocket
expenses will be determined by your health plan when they process
our claim. We do bill secondary insurance, so please inform us when
scheduling if you have secondary insurance.
Self Pay:
If you are paying for our services yourself, we require a minimum
payment of 50% three days prior to your appointment, and we will
bill you for the balance after your appointment. Our billing department
will contact you for payment arrangements.
Lawsuits and Settlements:
IPM does not place liens or accept guarantees of payment out of
legal settlements. There are no exceptions to this policy under
any circumstance. Patients are responsible to make timely payments
on their accounts.
Delinquent Accounts:
Payment of any amount for which you are responsible is due within
30 days of the date of our bill. Any balance over 30 days will be
considered past due and will accrue finance charges. If any portion
of the account reaches 90 days past due, the entire balance of the
account will be assigned to a collection agency for recovery.
